People go home and are expected to navigate complicated healthcare systems largely on their own. Popula was built to help close that gap.

Our story.
Popula was created from a simple observation: the healthcare system often focuses intensely on what happens during an appointment, but many of the things determining whether someone actually gets better happen afterward.
People leave the doctor's office with new medications, referrals, follow-up appointments, and discharge instructions — and are largely expected to navigate all of it on their own. For older adults, people managing complex conditions, and family caregivers, that burden can be overwhelming.
We built Popula to be the connective layer between people and the healthcare system — combining technology that helps identify what needs attention with registered nurses who understand what to do about it.

Popula is a care navigation and connected monitoring service — not a clinical provider. Our registered nurses work within a defined scope: organizing, navigating, monitoring, and escalating appropriately to the member's established healthcare team. We do not diagnose, prescribe, or replace any member's existing healthcare providers.
