What is Popula?

Popula is a nurse-led connected care company. We provide care navigation, connected health monitoring, and proactive follow-up support for people between medical visits. We work alongside your existing healthcare team — we don't replace your doctor, specialist, or emergency services.

Is Popula a healthcare provider?

No. Popula is not a clinical provider. Our nurses do not diagnose conditions, prescribe medications, or provide clinical treatment. We provide navigation, coordination, monitoring, and follow-through support. Clinical decisions stay with your healthcare team.

Question 3

Is Popula an emergency service?

No. Popula does not provide continuous real-time monitoring and is not an emergency-response service. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.

Who are Popula's care navigators?

All Popula care navigators are registered nurses. They are dedicated professionals — not call-center staff — who build ongoing relationships with the people they support.

Does Popula replace my doctor or care team?

No. Popula works alongside your existing healthcare team. We help make sure the care they've ordered actually happens — by navigating, coordinating, and following through on your behalf.

What does care navigation include?

Care navigation includes an initial nurse-led assessment, a dedicated care navigator, plain-language care plan education, provider and medication organization, referral and appointment coordination, barrier identification and resolution, community resource connection, and secure nonurgent messaging.

What is connected monitoring?

Connected monitoring uses home health devices (blood pressure cuff, pulse oximeter, glucose monitor, weight scale, etc.) to transmit health readings between visits. A Popula nurse reviews the information and reaches out when something needs attention. Connected monitoring is used when clinically appropriate and is not included in all plans.

How does Popula access my healthcare records?

Popula only accesses healthcare information you authorize. With your permission, we can request available records from your providers and organize them into a clear view. Your records are used to support your care navigation — not shared, sold, or used for any other purpose.

Question 9

Does Popula support pregnancy and postpartum care?

Accepted Answer

Yes. Popula supports prenatal navigation, connected monitoring for high-risk pregnancies when clinically appropriate, postpartum follow-up, and navigation for postpartum mood concerns. Popula does not provide obstetric care, labor support, or emergency services.

Can Popula help with post-hospital recovery?

Yes. Popula provides post-hospital transition support — helping bridge the gap between discharge and completed outpatient care, including referral tracking, appointment scheduling, and barrier resolution.

How much does Popula cost?

Popula offers three membership plans: Navigate ($149/month), Connected ($249/month), and Complete ($449/month). Each plan includes care navigation. Connected monitoring and higher-touch support are available in Connected and Complete plans.

Question 12

Does insurance cover Popula?

Popula is a direct-pay membership service. We are not currently in-network with insurance plans. However, Popula services may be eligible for reimbursement through HSA or FSA accounts. We provide superbill documentation to support out-of-network reimbursement requests.

What is a superbill?

A superbill is an itemized receipt that includes the information insurance companies need to process out-of-network reimbursement claims. Popula provides superbill documentation for members who wish to submit for potential reimbursement.

Can I cancel my membership?

Yes. Popula memberships can be cancelled at any time. See our Cancellation Policy for details.

Is there a contract or commitment?

Popula memberships are month-to-month with no long-term contract required.

Is Popula HIPAA-compliant?

Yes. All Popula systems and communications are HIPAA-compliant. Your health information is handled in accordance with applicable federal and state privacy laws.

Who can see my health information?

Your health information is only accessible to your Popula care team and, with your authorization, shared with your healthcare providers as needed to support your care. Popula does not sell, rent, or share your health information for marketing or commercial purposes.

How do I request my records or exercise my privacy rights?

Does Popula work with health systems and payers?

Accepted Answer

Yes. Popula offers population health and value-based care solutions for health systems, payers, and employer groups. Our organizational model is designed to support care gap closure, readmission reduction, and chronic condition management at scale.

What commercial models does Popula offer for organizations?
