You have the right to get a copy of your health and claims records, correct your records, request confidential communications, ask us to limit the information we share, get a list of those with whom we have shared information, get a copy of this privacy notice, choose someone to act for you, and file a complaint if you believe your privacy rights have been violated.
Our uses and disclosures
Popula may use or share your health information to provide care navigation and connected monitoring services, coordinate with your healthcare providers (with your authorization), and comply with legal requirements. We will not use or share your information other than as described in our complete Notice of Privacy Practices.
How to exercise your rights
